Forms & Payment

Forms & Documents

After setting up your first appointment, Comfort will email you forms to fill out. Additionally, please provide copies of the following documents to the SLP at your child's first session:

  • Current IEP

  • Speech and language evaluations

  • Hearing tests

  • Recent medical physical, and/or relevant medical evaluations

  • Goals that are currently/were previously targeted in therapy

    • To include physical therapy, behavior intervention, occupational therapy, and/or other speech services


Comfort provides superbills to families which can be submitted to insurance companies for reimbursement.  However, this will be a higher rate as it acts as an insurance claim, and with insurance claims we are required to bill all insurances the same rate, and this applies to the provision of a superbill. Your insurance carrier may request additional documentation from me to support the submitted claims. The rate billed to insurance will be $140.00. Comfort can contact your insurance carrier to determine out-of-network coverage and help you determine if the discounted private pay rate or using insurance is more advantageous. 


Payment is due at the date of service. We currently accept the following forms of payment: 

  • Cash

  • Credit Card

  • HSA Card
